showed pulmonary congestion and transthoracic echocardiography showed aortic valve regurgitation without ascending aortic dissection. Computed tomography (CT) demonstrated aortic dissection from the aortic arch to the bilateral common iliac arteries, but there was no evidence of dissection in the ascending aorta (Fig. 1A, B). A transesophageal echocardiography was performed and revealed a circumferential intimal flap with valve-like motion in the dilated aortic root (Fig. 1 C, D) and severe aortic valve regurgitation. Urgently, the Bentall procedure was performed and about 3/4 circumferential dissection above the sinus of Valsalva was confirmed. CT is selected most frequently as the initial test of acute aortic dissection, but there are some variants of aortic dissection which are not visualized on CT scans. Transesophageal echocardiography is useful for the diagnosis and it is reported to be

Extremely localized aortic dissection and intussusception of the intimal flap into the left ventricle.

Stanford type A aortic dissection frequently deforms the aortic root and causes aortic regurgitation (AR). On the rare occasion, massive AR can occur due to circumferential intimal disruption and prolapse of the cylinder-shaped intimal flap into the left ventricle. Because of the critical, general, and hemodynamic state of such patients, surgery for this condition carries a high risk. A 62-year...

Acute aortic dissection with intimal intussusception: transesophageal echocardiographic appearances.

A 71-yr-old man presented to our emergency department, with 12 hr of chest pain and dyspnea. A computed tomography scan demonstrated the presence of a 7.4 cm ascending aortic aneurysm, type A aortic dissection, in the distal ascending aorta extending into brachiocephalic and right common carotid arteries. A transthoracic echocardiogram showed type A dissection, with intimal flap prolapsing thro...

Aortic regurgitation secondary to back-and-forth intimal flap movement of acute type A dissection.

We present an unusual case of acute type A dissection complicated with severe aortic valve insufficiency caused by prolapse of the tubular intimal flap into the left ventricular outflow tract, which was shown legibly by transesophageal echocardiography in the diastolic phase and by intraoperative macroscopic findings. The dissected ascending aorta was excised completely and replaced without any...

Intimal tear without hematoma: an important variant of aortic dissection that can elude current imaging techniques.

BACKGROUND The modern imaging techniques of transesophageal echocardiography, CT, and MRI are reported to have up to 100% sensitivity in detecting the classic class of aortic dissection; however, anecdotal reports of patient deaths from a missed diagnosis of subtle classes of variants are increasingly being noted. METHODS AND RESULTS In a series of 181 consecutive patients who had ascending o...

A case of circumferential type A aortic dissection with intimal intussusception diagnosed using repeat transthoracic echocardiography examination

Case Sometimes it is difficult to diagnose circumferential aortic dissection with enhanced computed tomography alone. A 58-year-old woman presented with sudden-onset chest discomfort and loss of consciousness. Transthoracic echocardiogram showed mild aortic regurgitation. Enhanced computed tomography scans showed no obvious intimal tear or flap at the proximal ascending aorta, but an intimal fl...
